The Clinical Placement Reporting Tool
The School of Medicine’s Clinical Reporting Tool is the system by which students and placement staff can submit praise or any concerns they may have about the time a student has spent on placement. The School uses this process to locate areas of good practice, as well as address any issues that arise during placements.

Students can submit praise for a placement staff member, a team or a placement provider |
A placement can submit praise for a student, leading to a Clinical Placement commendation |
Amber |
Students can submit a minor concern about a placement staff member, a team or a placement provider |
A placement can submit a minor concern about a student |
Red |
Students can submit a major concern about a placement staff member, a team or a placement provider |
A placement can submit a major concern about a student |
Where Green cards are submitted, recipients of this praise will be told by the School with details of the praise being passed along to the student or placement provider staff member/teams.
Where Amber or Red cards are submitted, the concerns raised will be investigated within the School, where appropriate actions and outcomes will be decided.

Making a submission
*Please note, if you wish to submit multipe students for the same colour card, this will have to be done by completing individual forms. There is an option at the end of the form to submit another.
