Postgraduate research forms

There are a number of forms available for research staff and students to complete tasks in a number of areas.

Induction, progress and monitoring

Training Plan

Completed by student & supervisor within one month of the date of commencement of study

First Formal Progress Report  First Formal Progress Report to be completed by month 6 for full time candidates/ month 9 for part-time candidates/ month 18 for candidates on an Integrated degree of PhD and Masters. Once completed the form must be uploaded to the PDR. Student & Supervisor(s)
Progress Report Transfer Stage Must be completed prior to the Transfer review (Oral Examination). Once completed the form must be uploaded to the PDR. Student & Supervisor(s)

Joint Report of the Transfer Assessment Panel

Form to be completed by the Transfer Assessment Panel and uploaded onto the PDR. Once authorised by the PGR Tutor this will be released to PGR & Ops recommending action following transfer interview Transfer Assessment Panel

Annual Progress Review (Post-Transfer) Report Form

Form to be completed by the student and supervisor / independant assessor and uploaded onto the PDR Student & Supervisor / Independant Assessor
UAPP: Form for Instigation of Stage One

UAPP: Form to request immediate withdrawal on the grounds of unsatisfactory academic progress
Form to be completed by the Unsatisfactory Academic Progress Procedure Panel. Once authorised by the PGR Tutor or Head of School this should be uploaded to the PDR and PGR & Operations notified. Postgraduate Research Tutor or Head of School
Statement of Academic Integrity, Safeguarding Data and Ethical Requirements  (for Transfer Assessment) To be completed by the student and submitted with their transfer report submission. Student

To be submitted with the final corrected hard bound theses by any student not required to deposit an eThesis.

Students depositing an eThesis will instead complete an online version of this form as part of the eThesis upload process.

Awarded Statement

Awarded Statement 

Award statements are free of charge to former students for 3 months after they have appeared on a pass list. After the 3-month deadline, award statements will be £5 for the first copy and £3 per additional copy ordered (if ordered at the same time as the first copy). Those who qualify for a free statement should continue to request statements by downloading this form and sending to rp_examinations@leeds.ac.uk   or by completing a request form at the Student Services Centre counter. Please note the statement may take up to 5 working days to produce. Student
